La consommation nocive d\u2019alcool est d\u00e9finie diff\u00e9remment selon les pays, mais correspond en moyenne \u00e0 plus de deux consommations standard par jour, tant chez les hommes que chez les femmes (1).[\/vc_column_text][vc_single_image image=&#8221;49753&#8243; img_size=&#8221;large&#8221; alignment=&#8221;center&#8221; css=&#8221;&#8221;][vc_column_text css=&#8221;&#8221;]<\/p>\n<p style=\"text-align: center\"><em>Figure 1. Repr\u00e9sentation d\u2019une consommation standard d\u2019alcool (2)<\/em><\/p>\n<p>[\/vc_column_text][vc_column_text css=&#8221;&#8221;]La consommation r\u00e9cente d\u2019alcool peut \u00eatre d\u00e9tect\u00e9e par analyse de l\u2019air expir\u00e9, par la d\u00e9termination de l\u2019alcool\u00e9mie ou par la mesure de biomarqueurs directs dans le sang ou l\u2019urine, tels que l\u2019\u00e9thylglucuronide (EtG) ou l\u2019\u00e9thylsulfate (EtS).<\/p>\n<p>Les biomarqueurs indirects refl\u00e8tent les effets toxiques de l\u2019alcool sur les organes et les tissus, tels que l\u2019augmentation des enzymes h\u00e9patiques, l\u2019\u00e9l\u00e9vation de la transferrine d\u00e9ficiente en glucides (CDT), l\u2019augmentation du volume globulaire moyen (MCV) ou l\u2019\u00e9l\u00e9vation de la gamma-glutamyl-transpeptidase (GGT). Ces marqueurs indirects sont traditionnellement utilis\u00e9s pour d\u00e9tecter ou suivre un abus d\u2019alcool. Ils ne sont toutefois pas suffisamment sensibles pour d\u00e9tecter une consommation d\u2019alcool socialement acceptable. De plus, ils sont moins sp\u00e9cifiques, car des anomalies peuvent \u00e9galement \u00eatre dues \u00e0 d\u2019autres causes.<\/p>\n<p>La CDT est un exemple de biomarqueur indirect de l\u2019alcool. Elle est g\u00e9n\u00e9r\u00e9e par l\u2019inhibition du processus de glycosylation de la transferrine par l\u2019\u00e9thanol. La CDT est toujours rapport\u00e9e en pourcentage de la transferrine totale (%CDT). Chez les patients connus pour abus d\u2019alcool, la CDT est un marqueur tr\u00e8s sp\u00e9cifique, mais peu sensible pour \u00e9valuer ou suivre l\u2019abstinence. Le %CDT a une demi-vie de 2 \u00e0 3 semaines, mais n\u00e9cessite une consommation substantielle d\u2019\u00e9thanol (50 \u00e0 80 g\/jour pendant plusieurs semaines) avant de devenir positif. \u00c9tant donn\u00e9 que la production et la glycosylation de la transferrine ont lieu dans le foie, ce marqueur est peu fiable en cas d\u2019insuffisance h\u00e9patique s\u00e9v\u00e8re et de cirrhose. Des anomalies cong\u00e9nitales du processus de glycosylation et la grossesse peuvent \u00e9galement entra\u00eener des valeurs anormales (3).<\/p>\n<p>Un nouveau biomarqueur de l\u2019alcool, le phosphatidyl\u00e9thanol (PEth), est d\u00e8s lors de plus en plus utilis\u00e9 en alcoologie. Ce marqueur direct permet de distinguer l\u2019abus d\u2019alcool, le binge drinking et une consommation socialement acceptable de l\u2019abstinence.[\/vc_column_text][\/vc_column_inner][\/vc_row_inner][vc_row_inner el_id=&#8221;Phosphatidyl\u00e9thanol (PEth)&#8221;][vc_column_inner][vc_custom_heading text=&#8221;Phosphatidyl\u00e9thanol (PEth)&#8221; use_theme_fonts=&#8221;yes&#8221; css=&#8221;&#8221;][vc_column_text css=&#8221;&#8221;]Le phosphatidyl\u00e9thanol (PEth) regroupe une famille de phospholipides anormaux form\u00e9s exclusivement dans l\u2019organisme apr\u00e8s ingestion d\u2019\u00e9thanol. Le PEth s\u2019accumule en cas de consommation r\u00e9p\u00e9t\u00e9e d\u2019\u00e9thanol et est \u00e9limin\u00e9 lentement lors de l\u2019abstinence (3).<\/p>\n<p>Le PEth est form\u00e9 par l\u2019action enzymatique de la phospholipase D (PLD) sur la phosphatidylcholine (un lipide membranaire courant) en pr\u00e9sence d\u2019\u00e9thanol (Figure 2). La forte sp\u00e9cificit\u00e9 de la PLD pour l\u2019\u00e9thanol entra\u00eene la formation de PEth m\u00eame en pr\u00e9sence de faibles quantit\u00e9s d\u2019\u00e9thanol. Les mol\u00e9cules de PEth ainsi form\u00e9es s\u2019int\u00e8grent dans les membranes cellulaires, principalement dans les \u00e9rythrocytes, o\u00f9 elles sont relativement stables (3).<\/p>\n<p>La formation de PEth se produit dans diff\u00e9rentes cellules et tissus tels que les globules rouges, les plaquettes, les lymphocytes, le cerveau et le foie. Toutefois, l\u2019accumulation du PEth se produit uniquement dans les globules rouges, en raison de l\u2019inactivit\u00e9 de la phospholipase C, une enzyme importante dans la d\u00e9gradation des phospholipides.[\/vc_column_text][vc_single_image image=&#8221;49743&#8243; img_size=&#8221;large&#8221; alignment=&#8221;center&#8221; css=&#8221;&#8221;][vc_column_text css=&#8221;&#8221;]<\/p>\n<p style=\"text-align: center\"><em>Figure 2. Le PEth 16:0\/18:1 est la forme la plus fr\u00e9quente, car il est form\u00e9 \u00e0 partir de l\u2019homologue le plus abondant de la phosphatidylcholine dans la membrane des globules rouges (Figure 4b).[\/vc_column_text][vc_single_image image=&#8221;49747&#8243; img_size=&#8221;large&#8221; alignment=&#8221;center&#8221; css=&#8221;&#8221;][vc_column_text css=&#8221;&#8221;]<\/p>\n<p style=\"text-align: center\"><em>Figure 4a. Des exp\u00e9riences au cours desquelles des volontaires atteignaient une alcool\u00e9mie d\u00e9termin\u00e9e apr\u00e8s une consommation unique ou r\u00e9p\u00e9t\u00e9e ont \u00e9galement montr\u00e9 cette corr\u00e9lation positive entre le PEth et la quantit\u00e9 d\u2019alcool consomm\u00e9e.<\/p>\n<p>Cependant, des diff\u00e9rences interindividuelles significatives sont toujours observ\u00e9es entre les valeurs de PEth mesur\u00e9es en relation avec la consommation d\u2019alcool. Les causes possibles de cette variabilit\u00e9 interindividuelle incluent la vitesse de formation (vitesse de vidange gastrique, poids corporel, etc.) et la vitesse de d\u00e9gradation (activit\u00e9 enzymatique, etc.) (5).<\/p>\n<p>De mani\u00e8re g\u00e9n\u00e9rale, on consid\u00e8re que le PEth est form\u00e9 peu de temps apr\u00e8s l\u2019ingestion d\u2019\u00e9thanol et peut \u00eatre d\u00e9tect\u00e9 d\u00e8s 1 \u00e0 2 heures apr\u00e8s la consommation d\u2019alcool. Pour ce qui est de la vitesse de d\u00e9gradation, la demi-vie, est en moyenne de 7 jours pour le PEth 16:0\/18:1 (5). La concentration de PEth reste ainsi \u00e9lev\u00e9e pendant plusieurs jours \u00e0 plusieurs semaines.<\/p>\n<p>Une consommation quotidienne de 5 unit\u00e9s standard d\u2019alcool pendant 3 \u00e0 4 semaines entra\u00eene une augmentation de la concentration de PEth, d\u00e9tectable jusqu\u2019\u00e0 2 semaines apr\u00e8s le d\u00e9but de l\u2019abstinence (3).<\/p>\n<p>Ci-dessous, les r\u00e9sultats de plusieurs \u00e9tudes sont r\u00e9sum\u00e9s afin de donner une image r\u00e9aliste du PEth en relation avec la consommation d\u2019alcool.[\/vc_column_text][\/vc_column_inner][\/vc_row_inner][vc_row_inner css=&#8221;.vc_custom_1775210120167{margin-bottom: 0.625rem !important;}&#8221;][vc_column_inner width=&#8221;1\/2&#8243;][vc_custom_heading text=&#8221;Concentrations de PEth apr\u00e8s consommation d\u2019alcool&#8221; font_container=&#8221;tag:h4|text_align:left&#8221; use_theme_fonts=&#8221;yes&#8221; css=&#8221;&#8221;][vc_column_text css=&#8221;&#8221;]Dans une \u00e9tude exp\u00e9rimentale, 75 volontaires ont re\u00e7u 20 g d\u2019\u00e9thanol pendant 3 jours cons\u00e9cutifs. Des \u00e9chantillons sanguins ont \u00e9t\u00e9 collect\u00e9s le lendemain de la consommation. Une valeur maximale de PEth de 42,18 ng\/mL a \u00e9t\u00e9 mesur\u00e9e au jour 4. Dans une autre \u00e9tude, environ 20 \u00e0 65 g d\u2019\u00e9thanol ont \u00e9t\u00e9 administr\u00e9s \u00e0 un individu de 80 kg. Le PEth a pu \u00eatre d\u00e9tect\u00e9 dans le sang apr\u00e8s 30 minutes et la concentration maximale atteignait 147 ng\/mL. Des doses plus \u00e9lev\u00e9es d\u2019\u00e9thanol entra\u00eenent des valeurs de PEth plus \u00e9lev\u00e9es, mais toujours avec une grande variabilit\u00e9 interindividuelle, malgr\u00e9 l\u2019ajustement au poids corporel (6).[\/vc_column_text][\/vc_column_inner][vc_column_inner width=&#8221;1\/2&#8243;][vc_custom_heading text=&#8221;Les concentrations m\u00e9sur\u00e9es en s\u00e9rie de PEth permettent de diff\u00e9rencier les comportements de consommation&#8221; font_container=&#8221;tag:h4|text_align:left&#8221; use_theme_fonts=&#8221;yes&#8221; css=&#8221;&#8221;][vc_column_text css=&#8221;&#8221;]La mesure s\u00e9rielle du PEth permet de distinguer diff\u00e9rents modes de consommation d\u2019alcool, car ceux-ci influencent diff\u00e9remment la cin\u00e9tique du PEth. Une exposition r\u00e9p\u00e9t\u00e9e \u00e0 des concentrations moyennes d\u2019alcool entra\u00eene des valeurs de PEth diff\u00e9rentes dans le sang par rapport \u00e0 un \u00e9pisode unique de binge drinking, en raison de l\u2019accumulation du PEth et de sa demi-vie relativement longue. Des \u00e9tudes de dosage contr\u00f4l\u00e9 ont montr\u00e9 une augmentation progressive du PEth dans le sang avec l\u2019augmentation de la consommation hebdomadaire d\u2019alcool, toujours avec des diff\u00e9rences interindividuelles significatives (7).[\/vc_column_text][\/vc_column_inner][\/vc_row_inner][vc_row_inner css=&#8221;.vc_custom_1775210152973{margin-bottom: 0.625rem !important;}&#8221;][vc_column_inner][vc_single_image image=&#8221;49749&#8243; img_size=&#8221;large&#8221; alignment=&#8221;center&#8221; css=&#8221;&#8221;][vc_column_text css=&#8221;&#8221;]Figure 5a. Cette m\u00e9thode est consid\u00e9r\u00e9e comme le gold standard en raison de sa haute sensibilit\u00e9 et sp\u00e9cificit\u00e9. Elle permet de d\u00e9tecter et de quantifier le PEth \u00e0 partir de faibles concentrations.<\/p>\n<p>Un consensus r\u00e9cent propose un seuil de 20 ng\/mL comme indicateur d\u2019abstinence ou de consommation occasionnelle d\u2019alcool. Des concentrations &gt;200 ng\/mL sont suggestives d\u2019une consommation excessive ou chronique d\u2019alcool (8).[\/vc_column_text][vc_raw_html Une \u00e9tude contr\u00f4l\u00e9e men\u00e9e chez 16 volontaires utilisant un bain de bouche contenant de l\u2019\u00e9thanol 4 fois par jour pendant 12 jours cons\u00e9cutifs n\u2019a montr\u00e9 aucune valeur de PEth &gt;20 ng\/mL.
